The certificate control number
The single most common reason a teen road test gets turned away.
Look at the top of your certificate before you come
Your driver education certificate (DE-964 for teens, ADE-1317 for adults) must show its control number at the top. A certificate without one can’t be used for the road test, no matter which school issued it.
If you can’t find the number, call the school that issued the certificate before your appointment. It’s much easier to sort out in advance than on the day.
The same applies to the 30-hour log sheet. It has to be complete and signed — that’s the second most common reason a test doesn’t go ahead.
